The guy who roughed up Ryan is a murderer with an obstruction that will kill him if they don’t operate. They need to bring him back to the clinic. A longtime prisoner named Hugo must also be transported to the clinic, as the virus destroyed his liver. Hugo is a good guy whose chances for survival are looking pretty grim.
Otis realizes that Hugo carved a small eyehole into the women’s prison where he struck up a relationship with someone on the other side. That means there may be a hepatitis outbreak amongst the ladies, too. The woman Hugo’s been chatting with is an American named Becky. She’s serving 10 years for a one-time drug-trafficking mistake. Hugo befriended her when she arrived and gave her a reason to live. She’s never seen his face.
Hugo’s liver is shot and he’s going to die. Otis is able to swing it so Becky can visit him at the clinic. Unfortunately, Hugo is in a coma in post-op. In the next room, Ryan passes out during surgery on the killer con. The guard watching Becky leaves his post when he hears Ben yell for help. The guard helps save the patient and Ben is able to revive Ryan. Whew!
Becky takes advantage of the chaos to make a prison break. Otis wants Zee to cover for him while he does a hard target search for his fugitive patient. When he finds her, Otis tells Becky that Hugo is awake. She has to decide if she wants to run or go back so Hugo won’t die alone. Becky makes her choice. She’s by Hugo’s side as he watches the sunset he’s longed to see for the past 30 years.
Lily returns to the clinic after taking a few days off. Surprise, surprise—the first patient she treats is Mateo’s mom, Inez. Lily stabilizes Inez after she seizes. Now she just needs to determine what’s wrong with her ex-boyfriend’s cocaine-growing mama. When Inez seizes again, Lily drains fluid from around her heart and determines she has lupus. Inez later tells Lily the reason she got into the cocaine-growing biz is simple. It’s the only way she could make sure her children wouldn’t go hungry.
In random relationship news, Otis and Zee seem to be working through their differences, but they still have a ways to go. Lily knows Mateo’s mom will need long-term care, so she offers to be her personal physician. She doesn’t approve of what they do for a living, but knowing why they do it helps her deal. Ryan wants Ben to make her feel strong. He’s game at first, but not after what happened earlier. Ryan realizes that she can no longer practice field surgery. That’s not an easy thing to accept, but accept it she does.
As for Tommy and Mina, they are beyond awkward when around each other. Mina admits that she fears they messed up their friendship. That would be too bad because with Tommy and Lily, Mina feels like she’s home. We get the sense that Tommy feels the same way. The fact that they’re taking about it is a good sign. Now all Tommy has to do is stop picturing Mina naked.
Zee and Otis meet Ryan and Ben, who are returning from New York.
